Both Boeing 737 MAX 8 crashes were traced to a single flight-control system, compounded by commercial pressure to beat a rival and a breakdown in process discipline.

This wasn’t simply a software bug. It was an avoidable failure of process discipline, the kind of failure that aerospace quality standards such as AS9100 (built on the foundations of ISO 9001) exist to prevent. These standards demand rigorous design control, risk management, configuration management and verification precisely so that schedule and competitive pressure can never override safety.

The wider lesson applies well beyond aerospace: a management system only protects you if the process is genuinely followed, not bypassed when it becomes inconvenient. Strong quality management and effective internal auditing are how organisations catch shortcuts before they become catastrophes.
